Individual Health Card: How to Apply for the Healthcare Card

The Tarjeta Sanitaria Individual (TSI) gives access to public healthcare in Spain. Without it, consultations, tests, and medical prescriptions fall outside the scope of the SNS — the National Health System.

What is TSI

The TSI is issued by each Autonomous Community and provides access to all services of the SNS: family doctor, specialists, emergencies, discounted pharmacy, and hospitalization. It carries a unique and lifelong SNS code that identifies you in any community in Spain.

Starting from 2026, most communities also offer a digital version of the TSI via an app — useful for not having to carry the physical card.

Differences by Autonomous Community

The SNS is national, but each community issues its own TSI and may have small differences in the process:

Madrid: requires registration for a minimum of 3 months; online scheduling available on the Comunidad de Madrid portal
Andalucía: accepts applications in person or through the digital system Salud Responde; the TSV (digital version) has been available since March 2026
Catalonia: issued by CatSalut, with its own CIP number in addition to the SNS code
Valencia: via the Conselleria de Sanitat portal

In all communities, a TSI issued in one is valid nationwide — you can use it in any state of Spain.

Digital version

Most communities already offer the digital card through an app or web portal. Before downloading, check the official app for your community — each has its own (for example, Comunidad de Madrid for the capital, MySalud for the Valencian Community).
